Putting on a jacket, reaching into a cabinet, fastening a bra, washing your hair, or reaching into the back seat of your car may suddenly become much more difficult.



Then something else begins to happen. Your shoulder doesn't just hurt—it stops moving. This progressive combination of pain and stiffness is characteristic of adhesive capsulitis, commonly known as frozen shoulder. Unlike ordinary muscle tightness, frozen shoulder involves changes within the capsule surrounding the shoulder joint. Inflammation is thought to occur early in the process, followed by thickening, fibrosis, and contraction of the joint capsule. The result can be a substantial loss of shoulder mobility that lasts for months and, in some cases, years.


The encouraging news is that frozen shoulder usually improves. The frustrating part is that recovery can be slow. Understanding what is happening inside the shoulder—and recognizing that treatment should change depending on the stage of the condition—can make the recovery process much easier to navigate.



What Is Frozen Shoulder?


Your shoulder is a ball-and-socket joint. The rounded head of your upper-arm bone, or humerus, sits inside a relatively shallow socket. Surrounding this joint is a flexible capsule of connective tissue that helps provide stability while still allowing the shoulder's enormous range of motion.


With adhesive capsulitis, that capsule becomes inflamed and progressively fibrotic, thickened, and contracted. StatPearls describes changes involving inflammation, excessive collagen deposition, capsular thickening, and fibrosis, all of which contribute to restricted movement.


This explains something very important: frozen shoulder is not simply a tight shoulder muscle. You can certainly develop muscular tightness around the shoulder because you are moving less and compensating for the restriction, but the primary problem involves the shoulder joint capsule itself.


That distinction matters because repeatedly stretching or aggressively massaging surrounding muscles cannot simply force the condition to disappear. Treatment needs to respect the underlying condition and the stage of recovery.



The Hallmark Sign: Both Active and Passive Movement Become Restricted


One of the most useful ways to understand frozen shoulder is to compare active and passive range of motion. Active range of motion refers to how far you can move your arm yourself, while passive range of motion refers to how far someone else can move your relaxed arm.

Someone with certain rotator-cuff problems, for example, may struggle to actively raise the arm because of pain or weakness while still allowing considerably more movement when someone else moves the arm passively. With frozen shoulder, the joint itself becomes mechanically restricted.



What Does Frozen Shoulder Feel Like?


Frozen shoulder usually develops gradually rather than from one dramatic moment.

The combination of pain and progressively disappearing movement is often what eventually leads someone to seek treatment.



The Three Stages of Frozen Shoulder


Frozen shoulder is commonly described as progressing through three stages:


Freezing → Frozen → Thawing


These stages are useful because the shoulder may require a different approach depending on where you are in the process.


Stage 1: The Freezing Stage


The freezing stage is typically the most painful. Pain gradually increases while shoulder mobility begins to decrease. Night pain can be particularly frustrating, and sudden movements may produce sharp discomfort.


Studies estimates that this stage may last approximately six weeks to nine months, while other clinical references describe roughly two to nine months.


This stage is where people frequently make an understandable mistake. They feel the shoulder getting tighter and decide they need to stretch it harder. But an irritated shoulder does not necessarily respond well to aggressive stretching. During a highly painful freezing stage, repeatedly forcing the shoulder into severe pain may simply aggravate symptoms.


The objective is generally to maintain appropriate movement while respecting the shoulder's irritability. More aggressive does not automatically mean more effective.


Stage 2: The Frozen Stage


Eventually, something strange may happen. The shoulder may hurt less, but it can feel even more restricted. This is the frozen stage.


Studies estimates this phase at approximately two to six months, while StatPearls describes a broader range of roughly four to 12 months, illustrating how variable the condition can be.


Pain may become less dominant while stiffness becomes the primary limitation. This is often when people notice just how much motion they have lost. Because pain may be decreasing, appropriately progressive mobility work may become more tolerable. However, treatment should still be individualized.


Stage 3: The Thawing Stage


The final stage is the recovery or thawing phase. Pain continues to decrease, and shoulder movement gradually begins returning.


Cleveland Clinic estimates that this stage may last approximately six months to two years. StatPearls notes that the overall course can sometimes extend up to approximately three years.


That may sound discouraging, but it is important to understand that this doesn't necessarily mean someone experiences severe pain for three years. Symptoms change throughout the process. During the thawing phase, the focus can increasingly shift toward restoring mobility, rebuilding strength, and returning to normal activities.



Why Does Frozen Shoulder Happen?

Cedars-Sinai notes that primary adhesive capsulitis remains considered idiopathic, meaning the exact initiating cause isn't known, although inflammation appears to play an important role.


Clinicians generally divide frozen shoulder into two broad categories: primary and secondary adhesive capsulitis.


Primary Frozen Shoulder


Primary adhesive capsulitis develops without an obvious injury or triggering event. Someone may simply begin experiencing shoulder pain and progressive stiffness. There may be no fall, obvious muscle tear, or memorable injury. The condition simply develops.


Secondary Frozen Shoulder


Secondary adhesive capsulitis occurs following another event or condition that affects normal shoulder movement.

Potential triggers include:

  • Shoulder injury

  • Surgery

  • Prolonged immobilization

  • Extended periods of reduced arm movement

Studies specifically identifies trauma, surgery, and prolonged immobilization as important causes of secondary adhesive capsulitis. This is one reason restoring appropriate movement after an injury or surgery can be important once movement is medically permitted.



Who Is More Likely to Develop Frozen Shoulder?


Frozen shoulder can affect many people, but certain groups have a higher risk. It most commonly occurs during middle age and is more frequently reported in women. Diabetes is one of the strongest associations, and thyroid disorders are also associated with increased risk.

Other factors may include:

  • Previous shoulder injury

  • Shoulder or chest surgery

  • Prolonged immobilization

  • Certain systemic or neurological conditions

StatPearls notes that diabetes and thyroid dysfunction are particularly important because these conditions can also be associated with more severe or prolonged presentations.


This does not mean everyone with diabetes or thyroid disease will develop frozen shoulder. It simply means the association is strong enough to be clinically important.


Frozen Shoulder vs. Rotator Cuff Problems


One of the most common questions is, “How do I know whether I have frozen shoulder or a rotator-cuff problem?” The conditions can produce overlapping symptoms, including pain, difficulty reaching overhead, night discomfort, and reduced function.


However, there is an important difference. With adhesive capsulitis, there is typically a substantial restriction in both active and passive shoulder movement. With some rotator-cuff problems, active movement may be painful or weak while passive movement remains relatively better preserved.


That is not a perfect home diagnostic test. Shoulder conditions can overlap, and a painful shoulder can be difficult to examine accurately. Healthcare professionals may use physical examination and, when necessary, imaging to exclude other causes.


Mayo Clinic notes that frozen shoulder can often be diagnosed clinically, while X-rays, ultrasound, or MRI may be used to rule out other problems. StatPearls lists several conditions that can resemble adhesive capsulitis, including rotator-cuff impingement, biceps tendinopathy,

glenohumeral arthritis, cervical radiculopathy, calcific tendinitis, fracture, and other disorders.


That is why persistent shoulder pain should not automatically be self-diagnosed as frozen shoulder.



Why You Shouldn't Simply Force a Frozen Shoulder


When something feels tight, the natural instinct is to stretch harder. That approach can backfire with frozen shoulder, particularly during the painful stage.


There is an important difference between gently challenging restricted movement and repeatedly forcing an irritated joint far beyond what it currently tolerates. Exercise should be progressive. The objective is not to win a battle against the shoulder during every stretching session. It is to gradually improve movement without unnecessarily increasing pain and guarding.


Throughout more than 23 years of working with clients with shoulder problems, I have seen how easily people assume that a stiff shoulder simply needs more aggressive stretching. Frozen shoulder requires more patience.


Sometimes the best treatment is not the technique that creates the largest immediate stretch. It is the approach that allows the shoulder to gradually tolerate more movement over time.



What Actually Helps Frozen Shoulder?



There is no single treatment that immediately “unfreezes” every shoulder.

Management generally focuses on:

  1. Controlling pain

  2. Preserving or restoring appropriate range of motion

  3. Gradually rebuilding function

  4. Considering additional medical interventions when conservative treatment is insufficient

Mayo Clinic describes pain management and preservation of shoulder motion as the main goals of treatment. Physical therapy and regular range-of-motion exercise are commonly used.


Physical Therapy and Exercise



Exercise is one of the foundations of conservative frozen-shoulder management.


A physical therapist can help determine:

  • Which movements are restricted

  • How irritable the shoulder currently is

  • How aggressively it should be stretched

  • When strengthening should progress

  • Whether other shoulder or scapular muscles need attention


The exercise program should evolve as the shoulder changes. A highly painful freezing-stage shoulder should not necessarily be treated exactly like a shoulder well into the thawing phase.


Useful Frozen Shoulder Exercises


There is no universally perfect exercise program, but several simple movements are commonly used. The goal should be gentle, progressive restoration of motion rather than aggressive forcing.


1. Pendulum Exercise


Lean forward slightly while supporting yourself with the unaffected arm and allow the affected arm to hang relaxed. Use gentle body movement to allow the arm to swing forward and backward, side to side, and in small circles.


Mayo Clinic specifically includes pendulum movements among exercises that may help shoulder range of motion. The key is relaxation. This should not become an aggressive shoulder workout.


2. Wall Walk


Face a wall and place the fingers of the affected arm against it. Slowly “walk” your fingers upward, going only as high as you can comfortably tolerate. Pause briefly, then return.


Mayo Clinic also describes finger walking up a wall as a mobility exercise for frozen shoulder. Over time, you can monitor whether your hand gradually reaches higher.


3. Assisted Shoulder Elevation


Lie on your back or sit comfortably. Use the unaffected arm, a stick, or another appropriate aid to gently assist the affected arm upward. The unaffected side helps guide the movement rather than forcing it.


4. Gentle External Rotation


External rotation is often one of the most restricted movements in adhesive capsulitis. With the elbows near the body, a stick or dowel can be used to gently guide the affected forearm outward.


Move until you feel an appropriate stretch, but do not force the shoulder into severe pain.


5. Scapular Control


The shoulder blade is not the primary cause of adhesive capsulitis, but it often begins compensating for lost glenohumeral movement. Simple scapular-control and postural exercises can therefore be useful as part of the larger rehabilitation process.


As shoulder mobility improves, strengthening can gradually become more important.



Should Frozen Shoulder Exercises Hurt?


Some stretching discomfort may occur, but severe pain is different. This is where generic advice such as “push through it” becomes problematic.


The appropriate intensity depends on:


  • The stage of frozen shoulder

  • Current pain level

  • Irritability

  • Range of motion

  • Medical history

  • Individual response


If exercise repeatedly produces a significant flare that lasts long afterward, the program may need modification. The objective is progression—not punishment.



Can Massage Help Frozen Shoulder?



Massage can be useful, but its role needs to be understood correctly. Frozen shoulder involves the joint capsule. A massage therapist cannot simply massage through the deltoid and physically remove adhesive capsulitis.


However, months of altered shoulder movement can create substantial secondary muscular tension and compensation.


Muscles around the:


  • Neck

  • Upper back

  • Shoulder blade

  • Chest

  • Rotator cuff

  • Upper arm

may begin working differently because the shoulder joint itself is restricted.


Appropriate massage and manual therapy may help address some of this secondary muscular discomfort, improve general movement comfort, and complement an exercise or rehabilitation program.

What About Active Release Techniques?


Active Release Techniques may also have a role when surrounding muscles and other soft tissues have developed secondary restrictions. But the same principle applies.



ART cannot simply “break up” a frozen shoulder capsule and instantly restore normal motion. If muscles surrounding the shoulder, scapula, chest, or neck have become overworked because of compensation, targeted treatment may help those tissues.


The value comes from using ART appropriately within the larger rehabilitation plan—not trying to make ART replace the entire plan.


Heat or Cold?


Both heat and cold may be useful for symptom management. Mayo Clinic and Cleveland Clinic note that heat or cold can help manage shoulder discomfort.



Some people find gentle heat before mobility work makes movement more comfortable. Cold may feel better when the shoulder is irritated afterward. There is no need to turn either into a complicated treatment ritual. Use the option that helps you move and manage symptoms comfortably unless your healthcare professional advises otherwise.


Corticosteroid Injections


For some people, especially during painful stages, corticosteroid injections may be considered. Mayo Clinic notes that corticosteroid injections into the shoulder joint may help reduce pain and improve movement, particularly when administered relatively early in the condition.


The decision should be made with an appropriate healthcare professional based on symptoms, medical history, and the stage of the condition. An injection is not a substitute for restoring movement, but reducing pain may sometimes make rehabilitation more tolerable.


Hydrodilatation


Hydrodilatation is another treatment option. During the procedure, fluid is injected into the shoulder joint to expand and stretch the contracted capsule.


Mayo Clinic describes hydrodilatation as an option intended to make the joint easier to move and notes that it may sometimes be combined with a steroid injection. Not every person with frozen shoulder needs this procedure. It is one option within a broader treatment spectrum.



Manipulation Under Anesthesia and Surgery


Most cases are managed without surgery. However, persistent severe restriction that fails to improve with conservative treatment may lead to consideration of more invasive options.


These can include manipulation under anesthesia, where the shoulder is moved through ranges while the patient is anesthetized, or arthroscopic capsular release, where tightened portions of the joint capsule are surgically released.


Mayo Clinic emphasizes that surgery for frozen shoulder is relatively uncommon and generally considered when other approaches have not produced sufficient improvement.



Can Frozen Shoulder Be Prevented?


Not every case can be prevented. Primary adhesive capsulitis can appear without an obvious trigger, and its exact cause remains incompletely understood.


However, there is one modifiable risk factor worth emphasizing: prolonged immobilization. Following shoulder or upper-extremity injury or surgery, restoring appropriate movement when medically permitted may help reduce prolonged stiffness.


That does not mean moving an injured or postoperative shoulder before it is safe. Follow the rehabilitation restrictions provided by the treating healthcare team. The principle is simply to avoid unnecessary immobilization once movement is permitted.



How Long Does Frozen Shoulder Take to Heal?


This is often the question people most want answered. Unfortunately, there is no exact timeline. Frozen shoulder can take months to years to fully resolve.


Cleveland Clinic states that symptoms typically improve within approximately one to three years. StatPearls similarly notes that some people recover within months while others experience a prolonged course approaching three years.


The good news is that prognosis is generally favorable. StatPearls reports that approximately 80% of patients regain near-normal or normal shoulder function with appropriate treatment, although some people continue to experience residual stiffness or discomfort. Diabetes and thyroid dysfunction can be associated with a more difficult course.


Recovery is usually gradual. You may first notice a little less night pain, then slightly easier dressing, followed by a little more external rotation and eventually improved overhead reach. Progress may happen in small increments rather than one dramatic breakthrough.

Mistake #1: Stretching as Hard as Possible


More intensity does not automatically mean faster recovery.

The shoulder's irritability and stage matter.


Mistake #2: Completely Avoiding Movement


Fear of pain can lead people to stop using the arm entirely. Unless medically instructed otherwise, maintaining appropriate movement is generally part of conservative management. Mayo Clinic advises continuing to use the shoulder and arm as much as possible within pain and range-of-motion limitations.


Mistake #3: Treating Only the Muscles


Muscular compensation matters, but adhesive capsulitis primarily involves the joint capsule. Addressing only surrounding muscular tightness may therefore miss an important part of the problem.

That may involve a combination of:

  • Appropriate manual therapy

  • Gentle mobility

  • Soft-tissue treatment

  • Scapular work

  • Progressive strengthening

  • Movement education

  • Collaboration with physical therapists, physicians, or other healthcare professionals when necessary

As the condition changes, the treatment should change with it. During a highly painful freezing stage, symptom management and tolerable movement may be the priority. As pain settles and stiffness becomes dominant, progressive mobility can become increasingly important. During the thawing stage, strength, control, and return to normal activity become larger parts of the process.

The shoulder should not be treated according to a generic protocol. It should be treated according to the person and the stage of the condition.

Frozen shoulder is not simply a tight muscle that needs to be stretched harder. It is a condition involving real changes within the shoulder capsule, and recovery can take time.


But the overall prognosis is generally positive. With patience, appropriate treatment, progressive movement, and a rehabilitation strategy that respects the stage of the condition, most people can make substantial improvements in shoulder function.


The goal is not simply to make the shoulder temporarily feel looser. The goal is to restore enough comfortable, controlled movement to get back to the activities that matter.
