While that can certainly be true, it isn't the only possibility. In fact, not every case of sciatic nerve pain originates in the lumbar spine. For many people, the source of irritation may be much closer to the hip, specifically a small muscle deep within the buttock known as the piriformis. This is why understanding the difference between sciatica and piriformis syndrome is so important.
Although these two conditions often produce similar symptoms, they are not the same diagnosis. Treating them the same way can sometimes delay recovery or allow symptoms to persist longer than necessary.
What Is Sciatica?
One of the biggest misconceptions surrounding sciatica is that it is a diagnosis. It isn't. Sciatica is actually a description of symptoms, not the underlying cause. Although both individuals experience pain radiating down the leg, the underlying cause—and therefore the appropriate treatment—may be very different.
This is one reason why professional assessment is so valuable. Rather than assuming all sciatic pain comes from the spine, it's important to determine what is actually irritating the nerve.
Understanding the Sciatic Nerve

The sciatic nerve is the largest and longest nerve in the human body. It begins in the lower spine, where several nerve roots exit the lumbar and sacral vertebrae before joining together to form one large nerve.
From there, it travels:
Through the pelvis
Beneath (or occasionally through) the piriformis muscle
Into the buttock
Down the back of the thigh
Behind the knee
Into the lower leg
Finally reaching the foot
Its primary job is to provide movement and sensation throughout much of the lower extremity.
Because the sciatic nerve travels such a long distance, irritation anywhere along its path can produce symptoms far away from the actual source. This helps explain why someone may feel pain in the calf or foot even though the underlying irritation occurs in the hip or lower back.
Understanding this concept often helps people appreciate why successful treatment requires identifying the true source of nerve irritation rather than simply treating wherever symptoms are felt.
What Is the Piriformis Muscle?
The piriformis is a small, pear-shaped muscle located deep within the buttock underneath the larger gluteal muscles. Although relatively small, it plays an important role in stabilizing the hip and controlling movement of the leg.

Its primary responsibilities include:
Assisting external rotation of the hip
Helping stabilize the hip during walking
Supporting balance while standing on one leg
Controlling movement during changes in direction
Assisting pelvic stability during daily activities
Every time you walk, climb stairs, run, or shift your weight from one leg to the other, the piriformis works alongside the gluteal muscles and other deep hip stabilizers to maintain efficient movement. When functioning normally, you rarely notice it. However, when the muscle becomes irritated, overworked, or excessively tight, it may begin placing pressure on the nearby sciatic nerve. This is where piriformis syndrome develops.
What Is Piriformis Syndrome?
Piriformis syndrome occurs when the piriformis muscle irritates or compresses the sciatic nerve, producing symptoms that closely resemble sciatica caused by lumbar spine conditions.
Common symptoms include:
Pain deep within the buttock
Pain while sitting
Pain when climbing stairs
Tenderness over the piriformis muscle
Pain that travels down the back of the leg
Tingling or numbness
Increased discomfort after prolonged sitting

The Piriformis Usually Isn’t the Only Problem
One of the biggest misconceptions about piriformis syndrome is that the piriformis muscle simply becomes "tight" for no reason. In reality, muscles rarely become dysfunctional without an underlying cause.
Like many muscles throughout the body, the piriformis often compensates when other parts of the movement system aren't functioning efficiently.

Instead of becoming tight for no reason, it often becomes overworked because surrounding muscles are no longer sharing the workload efficiently.
Looking Beyond the Piriformis: Why the Whole Movement System Matters
One of the biggest reasons piriformis syndrome can be frustrating is that many people focus entirely on the muscle itself:
They stretch it.
Massage it.
Foam roll it.
Use a massage gun.
Apply heat.
While these treatments often provide temporary relief, many people notice the discomfort gradually returns.
Why? Because the piriformis muscle is often responding to another problem rather than creating the problem itself. Think of the piriformis as one member of a much larger team. If everyone else on the team is doing their job properly, the piriformis performs its role efficiently without becoming overloaded. However, when neighboring muscles stop contributing, movement becomes inefficient. The piriformis begins working harder to stabilize the hip, eventually becoming fatigued, tight, or irritated.

Your body doesn't function as separate parts. Instead, every joint, muscle, and connective tissue works together as one continuous movement system known as the kinetic chain. Walking is a perfect example.

If one area stops moving efficiently, another area often compensates. Initially, these compensations may not cause symptoms. Over time, however, repeated compensation can overload muscles that were never intended to perform extra work. The piriformis is one of those muscles.
Weak Glutes: One of the Most Common Contributors
The gluteal muscles are among the strongest muscles in the human body.

When the gluteus maximus and gluteus medius fail to generate enough force, smaller muscles—including the piriformis—often compensate. Imagine asking a small assistant to do the work of an entire construction crew. Eventually, that assistant becomes overwhelmed. The same thing happens within your hip. Rather than simply becoming tight, the piriformis often becomes overworked because larger muscles aren't contributing enough.
One pattern Mark frequently observes during movement assessments is that clients experiencing recurring buttock pain often demonstrate poor glute activation during activities such as squatting, climbing stairs, or standing on one leg. Hip Mobility: Why the Piriformis Works Overtime
Healthy hips should move freely in multiple directions.
These include:
Flexion
Extension
Internal rotation
External rotation
Abduction
Adduction
When hip mobility becomes restricted, the body still has to accomplish everyday tasks. Instead of the hip moving normally, surrounding muscles begin compensating. The piriformis is commonly recruited during these compensations.
For example:
If hip internal rotation becomes limited, the piriformis may remain active longer than necessary to help stabilize movement. Over weeks and months, this additional workload can contribute to fatigue, trigger points, and increased tension.
Many people are surprised to learn that improving hip mobility often reduces stress placed on the piriformis without directly treating the muscle itself.
Pelvic Alignment and Sacroiliac Joint Function
The pelvis serves as the bridge between the spine and lower extremities. Even subtle changes in pelvic movement can influence how muscles throughout the hips function. The sacroiliac (SI) joints connect the pelvis to the spine and help transfer forces between the upper and lower body during walking, lifting, and athletic activity.
When pelvic mechanics become altered—whether from previous injuries, muscular imbalance, prolonged sitting, pregnancy, or repetitive movements—the piriformis often helps stabilize the area. Although this compensation is beneficial initially, chronic overuse may eventually contribute to pain.
Why Sitting Makes Piriformis Syndrome Worse
One of the most common complaints among people with piriformis syndrome is: "It hurts most when I sit." There are several reasons why.
When sitting for prolonged periods:
The hips remain flexed.
The piriformis remains in a shortened position.
Blood flow decreases.

Because both conditions can produce pain traveling down the leg, they are frequently confused. Although symptoms overlap, there are often important differences.

Massage Therapy: Why It Helps And Why It Isn't the Whole Answer
Massage therapy can be extremely valuable for individuals experiencing piriformis syndrome and sciatic symptoms. When appropriate, techniques such as sports massage, trigger point therapy, myofascial release and deep tissue massage may help:
Reduce muscle guarding
Improve circulation
Decrease trigger point sensitivity
Improve tissue mobility
Promote relaxation
Reduce pain
Many people notice immediate relief following treatment. However, massage alone doesn't always prevent symptoms from returning. If weak glutes, poor hip mechanics, limited mobility, or altered movement patterns remain unchanged, the piriformis often becomes overloaded again.

One of the first things people search for after developing piriformis syndrome is: "What stretch should I do?" Stretching certainly has its place, and for many people it provides temporary relief. However, if stretching were enough to solve the problem, piriformis syndrome wouldn't keep coming back.

This is one of the biggest misconceptions surrounding hip and sciatic pain.
The piriformis muscle often becomes tight because it has been working too hard for too long. Simply stretching an overworked muscle without addressing why it became overloaded is similar to taking pain medication for a recurring headache without investigating what is causing it.
Long-term recovery usually requires a combination of:
Reducing irritation
Restoring hip mobility
Improving glute activation
Strengthening the hip stabilizers
Improving core control
Correcting movement patterns
Gradually returning to normal activity
Rather than relying on one exercise or one treatment, successful rehabilitation usually focuses on improving how the entire movement system works together.
Phase One: Calm the Irritated Tissue
When symptoms are severe, the first priority is reducing irritation and inflammation around the sciatic nerve. During this phase, less is often more.

Many people make the mistake of aggressively stretching an already irritated piriformis muscle. If stretching significantly increases numbness, tingling, or shooting pain, the tissue may simply need time to calm down before progressing into more intensive mobility work.
Successful rehabilitation is rarely about pushing through pain. Instead, it's about gradually improving movement while respecting the body's healing process.
Phase Two: Restore Healthy Hip Mobility
Once pain begins to settle, restoring normal hip motion becomes an important priority. Remember, the piriformis frequently compensates because the hip itself isn't moving efficiently.
Keep in mind that not everyone needs to do this. It is possible that a lack of hip mobility is NOT one of the issues causing your Piriformis Syndrome. That's why the diagnostic and assessment process can be so helpful, especially if your symptoms are severe and/or chronic. A thorough and accurate assessment helps you understand the unique characteristics of your body so you can employ a targeted strategy to obtain timely and sustainable pain relief.
Improving hip mobility allows larger muscles to resume their normal roles. Some effective mobility exercises include:
Figure Four Stretch
One of the most well-known piriformis stretches. This exercise gently lengthens the deep hip muscles while improving external hip rotation. Rather than forcing the stretch, gradual, comfortable movement generally produces better results.
Hip Internal Rotation Stretch / Band Mobilization
Many individuals with piriformis syndrome actually demonstrate limited internal rotation. Improving movement in this direction often reduces the workload placed on the piriformis during walking and daily activities.
90/90 Hip Mobility Drill
This exercise improves both internal and external rotation while teaching the hips to move independently from the lower back. It is particularly useful for individuals who spend long hours sitting.
Dynamic Hip Flexor Stretch
Tight hip flexors frequently accompany prolonged sitting. Lengthening these muscles may improve pelvic position while allowing the gluteal muscles to activate more effectively.
Phase Three: Wake Up the Glutes
If the gluteal muscles remain weak, the piriformis often continues compensating regardless of how much stretching is performed. This is why glute activation exercises are such an important part of rehabilitation. The goal isn't simply building stronger muscles. It's teaching the correct muscles to activate at the correct time.
Most people have some weakness, deficiency or asymmetry in their Glute Muscles. Mark has rarely found clients for whom this does not apply when there is a hip or pelvis injury of any kind. And while the solution may not be a one-size-fits-all approach, here are some basic Glute muscle exercises to get you started.
Glute Bridges
Glute bridges help restore hip extension while encouraging the gluteus maximus to perform its primary role. A common mistake is allowing the hamstrings to perform most of the work.Focusing on squeezing the glutes rather than simply lifting the hips often improves exercise quality.
Clamshells
Although simple, clamshells effectively strengthen the gluteus medius and deep hip stabilizers.
These muscles play an important role in controlling pelvic stability during walking and standing on one leg. Quality is far more important than speed.
Side-Lying Hip Abduction
This exercise targets the gluteus medius while improving lateral hip stability. Strong hip abductors help reduce unnecessary stress on the piriformis during walking, running, and climbing stairs.
Monster Walks
Using a resistance band around the knees or ankles, monster walks challenge the hips to stabilize while moving. They closely resemble the demands placed on the hips during everyday activities and sports.

Phase Four: Build Stability Throughout the Entire Body
The hips don't function independently. They rely heavily on the core, pelvis, and trunk to provide stability. Improving trunk control often reduces unnecessary compensation throughout the lower body.
As with the Glute muscles, there is no shortage of exercise options for strengthening the abdominal muscles and creating core stability. Every person has specific needs based on their own history and individual patterns.
An exhaustive list of exercises would require a very lengthy article with almost infinite explanations for why someone would need to do each and any of them. With that said, here are some basic ones that might be worth trying. As always, if you experience any pain or have trouble understanding technique, the best next step is to seek professional guidance.
Dead Bugs
Dead bugs improve coordination between the core, pelvis, and hips. Rather than emphasizing abdominal strength alone, they teach controlled movement while maintaining spinal stability.
Bird Dogs
Bird dogs challenge balance while strengthening the muscles responsible for controlling the spine and pelvis. This exercise closely mimics the coordination required during walking.
Side Planks
Side planks strengthen the lateral core muscles while improving pelvic stability. Strong lateral stability often reduces excessive hip movement during everyday activities.
Farmer Carries
One of the most functional exercises available. Farmer carries challenge the entire kinetic chain while reinforcing upright posture, core stability, and efficient walking mechanics.

Massage therapy is often one of the first treatments people seek when experiencing piriformis syndrome. It's important to ensure the massage therapist you choose is licensed and experienced in therapeutic massage. They should understand your injury, ask you questions, be able to talk competently about the anatomy and should be communicative with you during the session.
When performed appropriately, massage can help:
Reduce muscle guarding
Improve circulation
Decrease trigger point sensitivity
Improve soft tissue mobility
Reduce pain
Promote relaxation
Sports massage and trigger point therapy are particularly valuable because they address restrictions within the deep gluteal muscles surrounding the piriformis. However, massage works best as part of a comprehensive rehabilitation program.

Self-Massage / Trigger Point Release for Piriformis Syndrome
There is a plethora of self-massage tools out there that you have likely seen on the market for relieving muscle pain and trigger points. From theraguns to foam rollers to trigger point balls, there’s no shortage of ways to massage deep into your own Piriformis and Glut muscles for some at-home TLC.
While this can be an effective and cost-saving way to help move along the recovery process, it is important take it slow and not be too aggressive. Overdoing can irritate the Piriformis and things back.
Here are some basic guidelines for self-massage best practices:
Take it easy. Less is more at first, especially if you have a lot of pain. It's probably best to avoid self-massage during the acute phase, meaning if there are severe symptoms.
Only fold for 30-60 seconds in any one spot.
Hold still in each spot rather than rubbing back and forth. Using friction motions over tight muscles and trigger points can overstimulate and inflame muscles, especially muscles with recent injury history.
If you're symptoms worsen afterwards, do not continue with this strategy for the time-being.
If you experience any numbing or tingling down your leg during the self-massage, move off the spot immediately. You can try a nearby area but this may mean the entire region is still too inflamed.
